Immunization Requirements
The Colorado Department of Public Health and Environment has added the Tdap vaccine requirement for all 6th & 10th graders beginning in the 2007-2008 school year. This vaccine covers Tetanus, Diptheria and Pertussis. Pertussis or whooping cough can be very serious diseases in infants and small children. It has been found that frequently, siblings who have Pertussis disease can expose other children in the household. Colorado ranks fifth of all states in the country with this disease. The CDC recommends all 11 to 12 year old adolescents receive a dose of Tdap routinely.
Contact school personnel if you would like to obtain a personal, religious or medical exemption. You may have your child immunized at his/her doctor’s office or at the Mesa County Health Department (248-6900).
Please Note: Colorado School Entrance Immunization Law requires all students to provide proof of immunizations to attend school. Failure to supply the necessary information may result in your student being suspended from school.
Please see Complete listing of required immunizations.
Mesa County Health Dept. Regulations & Guidelines
According to the policy of the Mesa County Health Department, certain over the counter medications may be administered to a student, under the supervision of the School Nurse. Listed in this file are guidelines for the types and amounts of medications allowed.
Please see Types & Medications Administered
Community Service Hours
All students are asked to record 10 hours of community service during the school year, half of which can be done during the summer months.
Beginning in the Fall 2009-2010, parents can now keep track of their child(ren)’s hours on a monthly log page (a copy was provided in your student’s Back to School packet) and then turn it into their child’s homeroom teacher by the 5th of the next month. Their teacher will then input the information into Parent’s Web so you can keep track of how many hours your child(ren) have completed. You can print more forms off the website or you can pick one up in the Administration office. Only one child should be listed per page, because the teachers only have access to the students that are in their homeroom class. Please do not e-mail your student’s hours to Mrs. Ballif or to your child’s teacher, we respectfully request that you use the above mentioned form. If you have any questions, please call Correne Coty at 243-1771, ext. 11.
